Well: Panel Advises Against Routine Treadmill Stress Tests

Every year, hundreds of thousands of older Americans get on a treadmill in a doctor?s office and walk or jog as an electrocardiogram monitors their heart function. But a growing number of medical authorities would like to make routine screening using the procedure, known as the treadmill or exercise stress test, largely a thing of the past.

On Monday an expert government panel, the United States Preventive Services Task Force, joined the call by recommending against routine testing with electrocardiograms, or EKGs, in people who have no known risk factors or symptoms of heart disease, like shortness of breath or chest pains.

The recommendations, published online in Annals of Internal Medicine, made the test the latest addition to an expanding list of once routine screening tools that have fallen out of favor. Earlier this year, the task force advised against regular screening with the prostate specific antigen, or P.S.A., blood test, long considered the gold standard for early detection of prostate cancer. The panel has also come out against measures like annual Pap smears for many women and regular mammograms for women in their 40s.

For people at higher risk of heart disease, the panel found there was ?insufficient evidence? to determine the benefits and harms of screening with the EKG test ? either at rest or during exercise ? and advised that it be considered case by case basis. In those instances, said Dr. Michael L. LeFevre, a vice chairman of the task force and a professor of family and community medicine at the University of Missouri, patients should discuss with their doctors their medical histories and circumstances ? including age, blood pressure and cholesterol levels, and lifestyle ? to determine whether an EKG test might make sense.

The usefulness of the stress test has been questioned for some time, and in April, a group of nine medical specialty boards included it on its list of 45 common tests and procedures that doctors should perform less often. Their reasoning, like that of the task force, was that problems associated with the test can outweigh its benefit in many people, perhaps even leading to unnecessary harm.

?In my own practice I?ve seen people who thought they shouldn?t be exercising anymore because someone put them on a treadmill and got an abnormal test result when in fact there was nothing wrong with them,? said Dr. LeFevre. ?People get a false positive and can end up having a reaction to the subsequent angiogram or having something bad happen. Those things do happen.?

An underlying point of the new recommendations is that the emphasis should no longer be on screening people without symptoms, but on keeping coronary artery disease from developing in the first place, said Dr. LeFevre. ?We need to control blood pressure and cholesterol and get people to stop smoking and start exercising,? he said.

Treadmill stress tests are relatively quick and cheap compared with more invasive tests, and some doctors believe they can identify people with narrowed arteries that put them at risk of having a first heart attack. But studies have shown that the cause of such heart attacks is usually the sudden rupturing of plaque, something the test is unable to predict. And when used on people who are at low risk for heart disease, false positive results can lead to unnecessary anxiety and more costly procedures, like a coronary angiogram.

Dr. Todd D. Miller, a cardiologist at the Mayo Clinic in Rochester, Minn., who has studied the test?s effectiveness, said there were plenty of anecdotal reports about the treadmill stress test uncovering undiagnosed cases of heart disease, but no prospective studies bearing that out. Nonetheless, he predicted the test would remain widely used, at least in the near term.

?It takes a long time to see recommendations implemented in practice, and some of these old habits die hard,? he said. ?If some major health insurer says, ?We aren?t paying for that anymore,? then it generally gets done less. But if it?s just a formal medical organization putting out a recommendation, that usually has much less impact than payment issues.?

(AP) ? In the hunt for campaign money, no distance is too far to travel, especially when the race between President Barack Obama and Mitt Romney is tight and likely to stay that way into the fall.

The Democratic president and his Republican challenger have been aggressively courting Americans living abroad at fundraisers held far beyond U.S. shores. Such efforts serve the dual purpose of raising money to pay for what may be the most expensive election in U.S. history, and galvanizing a largely untapped group of eligible voters.

The practice is legal and has been used for decades, said former Federal Election Commission Chairman David Mason.

Obama has raised nearly $600,000 from Americans abroad while Romney has brought in about $325,000, according to campaign finance records analyzed by the Center for Responsive Politics. Those figures don't include sums raised overseas by both party committees or Romney's take from a pair of fundraisers in London during his visit there last week.

The sums are just a fraction of the more than $300 million Obama has raised overall and the $155 million raised by Romney, but every penny counts in a race that is neck and neck, as recent polls have shown.

One of Romney's London fundraisers raised eyebrows because the guest list included executives from Barclays, which recently admitted that bank employees were involved in manipulating a key market index. U.S. and British agencies fined the bank nearly half a billion dollars and Bob Diamond, who resigned as CEO, pulled out of the fundraiser. He had already given Romney's campaign the maximum individual donation of $2,500.

A separate well of potential donors awaited Romney in Israel, the second stop on the former Massachusetts governor's three-country tour. But there too Romney attracted some unwanted attention when his campaign announced it would break with its own precedent by barring reporters from covering a fundraiser at a swanky Jerusalem hotel. A day later, his campaign reversed course and said reporters could cover Romney's remarks to donors.

An invitation for the Monday event put the minimum ticket price at $50,000 per couple ? plus a copy of a U.S. passport. Romney advisers said the event was expected to raise more than $1 million.

Romney has vigorously courted Jewish and pro-Israel voters at home and in the Jewish state, hoping to capitalize on the perception by some that Obama hasn't been supportive enough of the critical U.S. ally. A delegation of Romney supporters, including former Bush press secretary Ari Fleischer, recently went to Israel to register eligible U.S. voters.

Matthew Brooks, who heads the Republican Jewish Coalition and organized the trip, noted that those voters live in a dangerous region.

"Whoever is the president of the United States has a direct impact on their lives," he said.

Showing deference to Israel also could help Romney shore up support from Sheldon Adelson, an American casino mogul who has pledged to spend more than $100 million to defeat Obama. Adelson, who attended Romney's Sunday speech in Jerusalem, is a staunch Israel supporter and finances a widely read, free newspaper there.

Early one May morning in Hong Kong, about 50 Americans squeezed into a conference room where Romney was beamed in by video teleconference for a fundraiser, said William Strong, an executive at Morgan Stanley who lives in Asia and helped organize the event. It was late in the evening for Romney in the U.S., but he spoke and took questions for about 45 minutes.

Obama too has overseas fundraisers taking place in his absence. His surrogates have been seen at fundraisers in Shanghai, Paris and London. Next month, actor George Clooney will headline an Obama fundraiser in Geneva, Switzerland. Guests can attend for $1,000. A $5,000 donation includes a photo reception and a $30,000 contribution buys dinner for two.

Obama has raised at least $2 million through nine expatriates who appear on his most recent list of bundlers ? supporters who group together contributions from friends and colleagues. Five Americans with addresses in the United Kingdom are on the list, plus two from France and one each from China and Switzerland. Romney has not released a full list of his bundlers.

U.S. law bans foreigners from contributing to political campaigns, but American citizens and green-card holders can legally donate regardless of where they live, subject to the $2,500-per-person contribution limit that applies to those living stateside. Campaigns, the parties and political committees are expected to review their donors and reject any ineligible contributions.

Election law experts have warned that the proliferation of super PACs has made it impossible to tell whether foreign cash is flowing to the campaigns. The foreign money ban also applies to super PACs, but some of their money comes from vague corporate entities, obscuring the original funding source.

Two percent of the money super PACs raised this year came in the form of so-called "secret money" that can't be reasonably traced, according to a preliminary analysis from an upcoming report by Demos and the U.S. Public Interest Research Group. In addition to super PACs, nonprofit groups are also spending heavily on the 2012 race, campaign finance experts said, and those groups aren't required to disclose their donors as long as their political activities stay within certain limits.

"As long as there is secret money sloshing around in our national elections, the public simply has no way of knowing if illegal foreign money is working its way into influencing our presidential and congressional races," said Fred Wertheimer of Democracy 21, which advocates for campaign finance reform.

An Associated Press review of donations to major super PACs this year found 55 contributions that either listed no address or were missing the U.S. state. The vast majority appeared to be oversights or sloppy bookkeeping, rather than foreign contributions. The Red, White and Blue fund, a super PAC that backed Rick Santorum's campaign, in February returned a $50,000 donation from a British securities firm that would have violated the ban on foreign funds.

For instance, the Acura MDX can tow 5,000 pounds, while the RDX can only tow 1,500. The Chevrolet Avalanche 2WD can pull 8,100 pounds, but the 4WD version can only pull about 7,900. The Chevy Tahoe is a similar category, with 8,500 pounds for the 2WD version and 8,200 for the 4WD version. Things get even stranger with the Jeep Wrangler Unlimited. The Sport 4WD version can tow just 1,000 pounds, but the Sport 4WD with the 3.73:1 axle can pull 3,500 pounds.

As a general rule, 4WD vehicles tend to have a lower towing capacity, while smaller, compact SUVs might have next to no capacity at all. If you?re unsure about what your vehicle can pull, you have two resources available to you. Check your owner?s manual and see if it lists a towing capacity specific to your vehicle, or contact a dealership. Of course, there are some handy websites online that can help you figure out what you can pull, including authority sites like Edmunds.com and Car and Driver.

Tow Package

You also need to make sure that your SUV has a towing package installed. Contrary to popular belief, a lot of SUVs and trucks do not come with a tow hitch installed. You will also find that a full OEM towing package includes more than just a hitch ? transmission coolers are also important parts of the system to ensure that your vehicle stays in good shape. If you don?t have a tow package, contact your dealer or head to your local mechanic?s shop. As a note, some SUVs only have OEM packages available, while others only have aftermarket solutions. Work with an expert and find out what your options are.

Your Trailer Makes a Difference

Towing watercraft requires more than just ?any old trailer?. Your trailer is going to be a very real investment, and you need to ensure that it stands the test of time and hard use. Make sure it?s certified by the National Marine Manufacturer?s Association. This ensures that the trailer is constructed to withstand the harsh environments near water sources, whether that?s a river, pond, lake or the ocean.

Offloading Your Watercraft

When you offload your watercraft, it?s best to do so from a boat ramp. This ensures that your boat (or other craft) has enough water beneath it to float fully and easily come clear of your trailer. It can also help avoid damaging underwater obstructions like rocks, tree trunks and the like. Always make sure that the area in which you offload is clear of obstructions and that there are no other boaters or swimmers in the vicinity before hitting the water.?

JEFFERSON ? The new small animal, rabbit and poultry exhibition barn at the Ashtabula County Fairground will be completed in time for the fair.

Dr. Becky Salinger, whose children are members of the All-American Animals 4-H Club, and who owns the Austinburg Veterinary Clinic, Austinburg, worked side-by-side with Debbie Platt to raise the $75,000 ?needed to build the barn.

There was a great need for a small animal and poultry barn; the previous areas were too small to accommodate all the poultry projects and there is no permanent structure for the turkeys, Salinger said.

The barn was raised completely on gifts by private donors.

The building is a pole building designed for the health and safety of the animals, the youth and the public, according to the Proposal for Ashtabula County Fairground Small Animal, Rabbit and Poultry Exhibition Barn and Educational Centers prepared by Ashtabula County Small Animal Committee and Agricultural Society booklet.?

It has three equally sized sections 48 feet long and 32 feet wide. This amounts to an overall building length of 168 feet with a width of 32 feet. The first section will house the chickens, ducks and fancy poultry projects. The second area will house the rabbits and the third, the turkey projects. For health reasons, chickens must be kept separate from turkeys.

?We have about 100 chickens entered in this year?s fair,? Platt said.

Specific features of the building include: cement floors with drainage for cleaning and disinfecting; side curtains versus solid walls to allow for better ventilation; ridge vent and fans for improved ventilation, and educational areas to divide the project areas and increase learning opportunities.

?It?s a great thing for the animals; it?s great for the children,? Platt said.

A few weeks ago, Williamsfield resident Steve Dillon and his 15-year-old daughter, Rebecca, took time to check out the new barn.?

A former member of the Beef-A-Teers 4-H Club, Dillon supports his daughter?s membership in 4-H and Future Farmers of America. She will show ducks and rabbits at the fair.

The NASA findings have caused some speculation in the media whether the "unprecedented" ice sheet melt is being caused by climate change, or as it is sometimes called, global warming. While NASA itself has not suggested that the ice sheet melt was caused by man caused global warming, which is said to be caused when carbon dioxide pumped into the atmosphere by power plants and factories trap the heat of the sun, a story in NPR was not so coy. NPR quotes other news outlets such as Wired and the UK Guardian that the phenomenon is evidence of global warming.

Climate change skeptics pounce

Fox News, on the other hand, quotes a number of climate change skeptics who suggest that fears that the Greenland ice sheet is being melted by man-caused global warming over overblown. Patrick J. Michaels, a climatologist, suggests in his blog that NASA is guilty of illiteracy by misusing the term "unprecedented" for something shown to have occurred every 150 years. As set forth in the Administrator's Complaint and as found by the Hearing Board, Respondent sought financial aid for his daughter's attendance at Frances Parker, a private school in Chicago for the 1999-2000 school year (Count I), the 2002-2003 school year (Count II) and the 2003-2004 school year (Count III). The school required parents applying for financial aid to submit an application, entitled Parents' Financial Statement (PFS). The statement requested information about the parents' income for the preceding year. Parents were also required to submit a copy of their federal income tax returns for the preceding year with the statement.

Respondent prepared and signed the Parents' Financial Statement for each of the years noted. Respondent falsely declared that the information in the document was true, accurate and complete. Respondent submitted false income amounts to Francis Parker on his PFS in order to be eligible for financial aid and with the intent to mislead the school. In addition, Respondent submitted to Francis Parker false income tax returns for the years 2000, 2001 and 2002, substantially understating the income he declared on his tax returns filed with the Internal Revenue Service for those years.

As to sanction:

Respondent engaged in serious misconduct. Over a period of several years, he provided financial aid applications to his daughter's school and included fraudulent tax returns in order to receive a financial benefit to which he was not entitled. As explained, he also failed to cooperate in the disciplinary investigation in many ways. Although Respondent contends that his behavior during the pre-hearing process and the hearing was proper and that he "fully participated in the process," the record demonstrates otherwise. An attorney's conduct in the disciplinary proceedings is a proper consideration when determining an appropriate sanction.
